Instructions for form submission:
Complete form and print a copy for your records.
After form completion click the "Submit by Email" button at the bottom of the form.
If you have an email program, such as, Outlook, Outlook Express, or Eudora, the form will automatically place an attachment to an email. Please send the email
If you use an internet email program, such as, Hotmail or Yahoo, please click the "Submit by Email" button and then choose to send via the Internet. Then click "Save Data File." Choose an appropriate location on your computer to save the data file, such as, the Desktop or My Documents. Open your Internet email, create an email to lwilson@starcouncil.org. Attach the data file you saved to the email and send.
Some people might have trouble sending the data file from their workplace depending on the network configuration. You will need Adobe Reader to complete the online application.
If you would like to download and mail the application please 'right-click' one of the documents below and chose 'save target as' to save to your computer. Then complete and mail the application to:
